Rehabilitation Nurse Practitioner

We are looking for a dedicated Rehabilitation Nurse Practitioner with a Suboxone waiver to become part of our team. The Nurse Practitioner will provide comprehensive care to adult patients facing substance use disorders and co-occurring mental health issues. You will assess patients for detox and rehabilitation, determine the appropriate care level, and address ongoing health concerns at our facility.

Key Responsibilities:

• Perform detailed evaluations, including patient history and health assessments based on current symptoms.
• Make differential diagnoses by reviewing clinical findings and patient histories.
• Conduct essential testing, such as urine and blood tests, along with glucose monitoring.
• Collaborate with physicians to create a personalized treatment plan that prioritizes the patient's needs.
• Implement care plans, including diagnostic tests, pharmacological interventions, and patient education, while making necessary referrals.
• Continuously monitor treatment effectiveness and modify plans to ensure optimal mental health outcomes.
• Provide educational materials to help patients make informed choices about health promotion, prevention, and treatment.
• Create a safe and supportive environment, acting as a patient advocate at all levels.
• Collaborate with healthcare professionals to deliver comprehensive and holistic care.
• Maintain accurate and confidential patient records in compliance with standards.

Qualifications:

• Master's Degree in Nursing.
• Current New York State Nurse Practitioner license.
• Registered NP certification with prescription privileges under NY law.
• DEA license and adherence to NY Department of Drug and Alcohol Program guidelines.
• Basic Life Support (BLS) certification.
• Five years of related work experience.
• ANCC certification and credentialing.
