Position Overview: Home Health LPN

Core Duties:

• Oversee the admissions, discharges, and transfers of residents.
• Manage medication requisitions with pharmacies and healthcare providers.
• Administer medications as prescribed.
• Follow established infection control procedures, emergency policies, and other facility standards.
• Work with the interdisciplinary team to ensure optimal outcomes for residents.
• Report any significant changes in residents' health to the appropriate staff as per policy.

Required Qualifications:

• Must hold a valid state-issued Licensed Practical Nurse (LPN) license.
• CPR certification is essential.
• Preference for candidates with two years of nursing experience.
