Urban Recovery specializes in providing care for individuals dealing with substance and alcohol use disorders. Our facility offers comprehensive services, including detoxification, medication-assisted treatment, rehabilitation, and recovery therapy. We are currently seeking Full-Time and Part-Time Care Coordinator (LPN) to join our team!

As LPN, you will deliver nursing care tailored to each client's needs, using established techniques and standards under the direction of a registered nurse. This position allows you to work collaboratively with other committed professionals while advancing your clinical skills!

Available Shifts:

• Day Shift: 7:00 AM - 7:30 PM
• Night Shift: 7:00 PM - 7:30 AM

Key Duties:

• Administer medications according to Safe Nursing Practice standards and physician orders.
• Educate patients on their medications, documenting their understanding and compliance; notify the registered nurse or physician of any irregularities.
• Document all patient activities, such as admissions, discharges, transfers, and emergency room visits; report any issues to the registered nurse or physician.
• Monitor and document the detoxification process and patient behavioral responses; communicate any concerns to the registered nurse or physician.
• Record treatment interventions and patient reactions, notifying the registered nurse or physician of any abnormalities.
• Collaborate with the treatment team regarding patient progress, reporting any concerns to the registered nurse or physician.
• Conduct withdrawal assessments and monitor vital signs as outlined in the treatment plan.
• Gather data for Continuous Quality Improvement (CQI) programs, including census, Kardex, risk management, incident reports, infection control, and medication event reports; inform the registered nurse or physician of any issues.

Qualifications:

• Graduate from an accredited nursing program.
• Current NY State Board of Nursing License and Registration.
• Current BLS/CPR certification.
• Minimum of two (2) years of nursing experience.
• Preferred experience in addiction treatment.
