Patient Case Manager

We are an organization that focuses on providing care and treatment for individuals struggling with substance and alcohol use disorders. We are looking for Patient Case Managers to join our team. In this role, Patient Case Managers will provide support to clients by removing obstacles to treatment and helping them remain engaged in addiction services. They will demonstrate recovery skills and coping strategies while inspiring hope in clients on their recovery journey.

Shifts Available:

• 3:00 PM - 11:00 PM
• 7:00 AM - 3:00 PM

Key Responsibilities:

• Interact with clients in a courteous, empathetic, and professional manner consistently.
• Communicate regularly with counselors and nursing staff about client care and issues.
• Promote positive interactions among clients and staff members.
• Assist with the transportation of clients throughout the facility.
• Ensure clients adhere to the scheduled program activities.
• Conduct room checks on all clients.
• Perform census checks every two hours (four times per shift), including after meals.
• Monitor clients for any inconsistent behavior and report to appropriate staff (Clinical, Nursing, etc.), including this information in your shift report.
• Assist in de-escalating situations involving clients exhibiting challenging behaviors.
• Oversee the management of client linen and laundry.
• Keep track of linen supplies and replenish as necessary.
• Collect dirty laundry and prepare it for housekeeping.
• Return clean laundry back to clients.
• Search and document the belongings of new admissions.
• Support staff in screening clients and their belongings for contraband.
• Act as a positive role model for both clients and staff!

Minimum Qualifications:

• High school diploma or equivalent.
• One year of relevant work experience.
