Position: Certified Practical Nurse (LPN)

Key Responsibilities:

Manage the admissions, discharges, and transfers of residents.
Coordinate medication orders with pharmacies and healthcare providers.
Provide medication administration as directed.
Comply with infection control standards, emergency policies, and other center protocols.
Work alongside the interdisciplinary team to promote optimal health outcomes for residents.
Identify and communicate any significant changes in residents’ conditions to the relevant personnel as per center policy.

Qualifications:

Must have a valid Licensed Practical Nurse (LPN) license issued by the state.
CPR certification is required.
Preferred candidates will have at least two years of nursing experience.
