Responsibilities for Home Health LPN:

• Facilitate admissions, discharges, and resident transfers
• Process medication requests with pharmacies and doctors
• Administer prescribed medications
• Comply with infection control and emergency policies
• Work closely with the interdisciplinary team to achieve optimal outcomes
• Monitor and report any unusual changes in residents' conditions as per center policies

LPN Qualifications:

• State licensure as a Licensed Practical Nurse (LPN) is required
• CPR certification is essential
• At least two years of prior nursing experience is preferred
