Health Services Coordinator (LPN) Responsibilities:

• Overseeing resident admissions, discharges, and transfers
• Handling medication requisitions with pharmacies and healthcare providers
• Administering medications as prescribed
• Following infection control protocols and other facility policies
• Collaborating with the interdisciplinary team to deliver the best care outcomes
• Reporting any significant changes in residents' conditions promptly

Licensed Practical Nurse (LPN) Qualifications:

• Valid LPN license from the State
• Current CPR Certification
• Preferably two years of nursing experience
